The District Health Visitor manages public health nursing in an assigned district, focusing on child health, health promotion, disease prevention, and rehabilitation within a primary healthcare framework.

 

JOB PURPOSE

The District Health Visitor is required to perform public health nursing within a primary health care paradigm and is responsible for the management of an assigned health district, towards optimum health and well-being of individuals, families and the community. The emphasis is child health, health promotion, disease prevention and rehabilitation.

The Authority reserves the right to assign the officer to any Facility under its management.

 

THE CORE FUNCTIONS INCLUDE BUT NOT LIMITED TO THE FOLLOWING:

  • Directs the activities of the health district.
  • Plays a central role in the management and delivery of public/ community health care.
  • Conducts population health assessments in health districts.
  • Provides antenatal care, intranatal and postnatal care to the child bearing family.
  • Provides expertise and guidance to all categories of nursing staff assigned to the health center.
  • Works closely with the Primary Care Physician and other members of the multi- disciplinary team to promote safe primary care services.
  • Plans, conducts and records home visits.
  • Supervises pupil midwives on home deliveries.
  • Ensures a robust communication system exists, which underpin service provision.
  • Provides telehealth and virtual health services to clients and their families.
  • Prepares the annual budget for the Health District.
  • Implements the operational plan for the Health District.
  • Maintains surveillance on critical areas that impact on health in the community.
  • Manages the immunization of the Health District.
  • Maintains the key performance indicators for primary care.
  • Maintains records and reports.
  • Plans and organizes regular meetings at the health centre.
  • Engages in professional growth and development.
  • Evaluates the performance of staff at the health centre.
  • Maintains records of shut-ins and community home residents in elder care or children’s homes.
  • Participates in planning and organizing Health Promotion/Education activities in conjunction with other stakeholders
  • Observes and implements policies in SWRHA especially relating to Out-Patient Clinic Management, Telehealth, Block Appointment, E-referrals and Patient Charter of Rights.
  • Performs any related functions which may be assigned from time to time.
  • Ability to perform duties according to assigned rosters or plans that can be formulated in times of authorized service exigencies.
  • Ability to use the relevant computer applications related to job functions.

 
Position Requirements

  • Experience in Public Health Nursing such as may have been gained in the next lower class and training as evidenced by the possession of certificates in General Nursing and Midwifery.
  • Diploma in Public Health nursing recognised by the Royal Society for the Promotion of Health.
  • Registration as a Nurse and as a Midwife by the Nursing Council of Trinidad and Tobago
  • Possession of the Royal Society of Health Certificate for Health Visitors and School Nurses.
