Under the direction and leadership of the Clinical Manager, the Community Psychiatric Nurse (CPN) will engage and support clients/patients and their families in the community, including clients’ homes.

 

Key responsibilities include:

  • Conducts mental health assessments/triage of the presenting symptoms of individuals referred to Outpatient Services
  • Manages assigned case load, conducts assessments and triage of referred patients/ clients, provides support to Clinic Triage Nurse (CTN) and supervises the work of support staff as applicable
  • Administers medications to patients/clients
  • Orders medications for patients/clients
  • Encourages support staff to deliver person-centered care that enables patients/clients  to engage in meaningful activities and relationships
  • Engages members of the public in the promotion of mental health/wellbeing and relapse prevention
  • Acts as Case Manager for assigned clients or groups of patients to develop and implement treatment plans and programs
  • Liaises with caregivers, relatives, and other care providers for best outcomes of patients/clients

 

Qualifications & Registration required:

  • Registered Psychiatric Nurse or equivalent with a minimum of three years post graduate experience
  • Certificate in Community Psychiatric Nursing or equivalent, preferred with three years of experience working in a community mental health setting
  • Current registration with the Bermuda Nursing Council
  • Valid car driver’s license
